Certified wound care nurses with advanced wound education, including WOCN’s in the East and West Valley, provide training, treatment, and wound consultations. Program for Type l and ll diabetic patients focusing on home diabetic care. Patients are provided personalized care, education on comorbidities, and instruction of disease process with goal to become independent and compliant in the self-management of their diabetes. In addition, nutritional education, lower extremity screening, and instruction on insulin and diabetic medications is emphasized.
