Orthopedic Home Rehabilitation
One on one therapy to focus on joint specific strengthening and education, gait training incorporating balance and coordination, pain management, and safety in the home environment. Protocols specific for every orthopedic physician and hospital through our trained therapy staff. Easy use coagulation therapy utilized to allow easier monitoring and instant results.
Certified Wound Care Team
Certified wound care nurses with advanced wound education, including WOCN’s in the East and West Valley, provide training, treatment, and wound consultations. Multidisciplinary support is available as necessary. Our certified wound nurse specialists are available for in-service education or consultative services to providers. Nurses have extensive knowledge in diabetic, venous stasis, surgical, and complicated wounds.
Program for Type l and ll diabetic patients focusing on home diabetic care. Patients are provided personalized care, education on comorbidities, and instruction of disease process with goal to become independent and compliant in the self-management of their diabetes. In addition, nutritional education, lower extremity screening, and instruction on insulin and diabetic medications is emphasized.
Nurses educate patients/caregivers on medication management, blood pressure monitoring, and importance of making heart healthy choices in diet and lifestyle. Therapists establish a home exercise program tailored to patient needs. Dietitians available to assist patients in understanding the particular benefits of diet management.
Cardiopulmonary - CHF, COPD
Comprehensive patient instruction in the home with identification of lifestyle risk factors, disease management, energy conservation methods, diet, and medications. Cutting edge technology monitors edema, daily weights, oxygen saturation levels, and blood pressure. Therapists educate on home exercise programs for endurance training. Our goal is patient compliance with their disease management.
Therapy staff trained in stroke recovery and assistive/adaptive devices will work with the patient to become as independent as possible in the home environment. Speech therapists utilize Vital Stim to allow the patient increased ability to swallow and communicate. Collaborative patient approach to ensure success with community integration is our goal.
Dementia Care Program
Nursing staff available for caregiver training, education on disease process, medication management, and home safety for dementia patients. Therapy specific training provided to caregivers for safe mobility techniques focusing on proper body mechanics. Therapists will assist with pain management, and establish an individualized home exercise program to maintain strength and flexibility.
Balance and Safety
This program is designed for those patients with a history of falls, gait disturbances, and recent decline in functional mobility. Our physical and occupational therapists utilize a comprehensive approach using interventions that focus on reducing fall risk while performing daily physical activities. Skilled nursing is available for medication reconciliation with the primary care physician to reduce side effects and drug to drug interactions that may contribute to balance impairments.
Assisteo’s Fall Prevention Program is designed for those patients with a history of falls and gait disturbances. Our therapists utilize a comprehensive approach that focus on reducing fall risk while preforming daily physical activities. A thorough balance assessment and home safety evaluation is completed to provide an individualized plan to address the deficits found.
